Position Summary: Rutgers, The State University of New Jersey is seeking a Community Coordinator in the Infectious Disease Division of the Eric B. Chandler Health Center at the Robert Wood Johnson Medical School.The Community Coordinator will be responsible for helping patients with HIV and their families to navigate and access community services, other resources and adopt healthy behaviors. The position supports medical and behavioral health care providers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain and improve the health of patients and their family.Among the key duties of this position are the following:Provide services to patients
